Over more than two decades in the U.S. Senate, Graham was a leading voice for maximum pressure on Iran, unconditional support for Israel, and military action in West Asia.

According to Axios, just hours before his death, Graham told those urging him to see a doctor that he could not die yet because he still had to push forward sanctions on Russia, resolve the Iran issue, and bring about normalization between Israel and Saudi Arabia. Shortly after, news of his death was announced, leaving these ambitions unfulfilled.

Graham was among the first to support the Trump administration's withdrawal from the Joint Comprehensive Plan of Action (JCPOA) in 2018. He repeatedly called for increasing sanctions, intensifying economic pressure, and even targeting Iran's vital infrastructure. He argued that the only way to confront Iran was through crippling sanctions and military threats. However, the maximum pressure policy did not achieve its stated goals of reducing Iran's oil exports to zero or forcing Tehran to change its behavior.

Graham was also an unconditional defender of Israel. He consistently supported Israeli military operations in Gaza, Lebanon, and Syria, and called for increased U.S. financial and military support for Israel. Israeli officials, including Prime Minister Benjamin Netanyahu, President Isaac Herzog, and others, mourned his death, calling him one of Israel's greatest friends.

In Washington, former President Donald Trump praised Graham as a true patriot, and the White House ordered flags to be flown at half-mast. Graham's death marks the end of a political era characterized by aggressive foreign policy, but many of his key objectives—such as regime change in Iran and isolating the country—remained unrealized. Iran continued to develop its defensive and regional capabilities, and expanded cooperation with Asian powers.
